Before You Begin, please complete this form only if you wish to request a formal Letter of Determinant Value from the AVOY Authenticity Division.

• This request will be reviewed by our team to assess eligibility for Letter of Determinant Value.
• If a Letter of Determinant Value can not be issued, this fee will be returned to you minus $50 for handling.
• Submission does not guarantee that a Letter of Determinant Value will be granted. If we can not confirm the authenticity of the work.

Ensure you have:

• High-resolution images of the front, back, and signature of the artwork
• Any prior ownership documentation or past COAs (optional, but helpful)
• Basic information about how and when the work was acquired
