“Important Information Before You Submit A Request for a Certificate of Authenticity (COA)”

Before You Begin, please complete this form only if you wish to request a formal Certificate of Authenticity (COA) from the AVOY Authenticity Division.

• This request will be reviewed by our team to assess eligibility for COA issuance.
• A nonrefundable review fee is required after submission.
• If a COA is issued, this fee will be credited toward the cost of your official certificate.
• Submission does not guarantee that a COA will be granted.

Ensure you have:

• High-resolution images of the front, back, and signature of the artwork
• Any prior ownership documentation or past COAs (optional, but helpful)
• Basic information about how and when the work was acquired
